Purpose of Form

Citizens requesting a Certified Death Certificate

1. You may print out the form and fill it in by hand or enter text directly into the form and print out.
2. Please be sure to sign the form when finished.
3. Upon completion of the form please mail or hand deliver to the address below, along with a copy of a valid State issued
Drivers License, valid State issued ID, or Military ID of the applicant requesting the Certified Death Certificate.
4. A notarized statement MUST accompany all "mail in" applications for a death certificate.

Processing Time: Processing will begin immediately upon receipt of the Death Certificate Application. Upon issuance of the Certified Death Certificate it will be mailed to the mailing address indicated on the Death Certificate Application by regular mail. If you would like to expedite the receipt of your Certified Death Certificate please coordinate shipping options through the City Secretary's office by calling (830) 774-8715.
